The Form’App platform is structured around 4 types of user:
- Method creator
- Method operator
- Consultant/trainer
- Beneficiary/trainee
Depending on how you use Form’App, you can select and combine different types of access.
Example : an independent consultant offering his own method can choose the role of Method Creator to design his training courses, the role of Method Operator to order and allocate the files, and then use the role of Consultant/Trainer to carry out the coaching himself.
Below is a description of each role and the costs associated with each access.
Method creator
This module allows you to create and update your own training courses or coaching sessions, known as methods.
You can also manage access to these methods for Method operators and validate access for Consultants/trainers recruited by such operators.
- 7.50 € excl. VAT per month for a subscription with a one-month commitment
- 75.00 € excl. VAT per year for a subscription with a one-year commitment
These prices include unlimited access to the module and an unlimited number of methods.

Method operator
This module lets you use the methods created by the Method creator.
You manage access for Consultants/trainers and Beneficiaries/trainees, order dossiers for coaching or training, and then allocate them to the users concerned.
The module also allows you to monitor the progress of these dossiers.
The price of a dossier is clearly indicated in the catalogue of available methods. It consists of a fixed price per dossier, plus a variable cost depending on the number of stages.
The dossier fee does not cover the cost of any rights to use a method, which are set at the discretion of the Method’s creator and invoiced directly by him/her.
- 2.50 € excl. VAT per dossier, fixed rate
- 10.00 € excl. VAT per stage, up to a maximum of 1 MB per stage
Example : For a dossier comprising 3 stages, each of a maximum of 1 MB, the total cost will be 2.50 € excl. VAT + (3 x 10.00 € excl. VAT), i.e. 32.50 € excl. VAT
Consultant/trainer
This module allows you to accept or refuse to accompany a beneficiary or to train a trainee.
Once the dossier has been accepted, it offers the option of scheduling sessions in advance or from one session to the next. It provides access to the next stage and adjusts the display of forms to personalise the method according to the customer’s needs.
Furthermore, it facilitates exchanges with the client via a logbook, enables the progress of the work to be monitored and, once validated by the beneficiary or trainee, enables additions or corrections to be made, in synchronous or asynchronous debriefings.
There is no charge for the Consultant/trainer. They have unlimited access to their dossiers.
Beneficiary/trainee
The Beneficiary/trainee role provides access to the dossier and allows following the coaching or training path proposed by the Consultant/trainer.
This module includes access to the explanations provided, the ability to carry out exercises, answer questionnaires, download images (in PNG or JPG format), communicate freely with the Consultant/trainer via a logbook, and to re-schedule appointments.
What’s more, the stages can be saved in PDF format for later storage outside the platform or for printing.
There is no charge for the Beneficiary/trainee. They have unlimited access to their own dossier.

The story of Form’App
The creation of Check-Up Pro, a Perigord-based project dedicated to skills assessment coaching, was an opportunity to design a scalable and adaptable support tool that can be accessed online.
In line with our values, the AC-IT team developed Form’App in collaboration with consultants specialising in skills assessment. Each feature has been designed to lighten the operational load on consultants, enabling them to concentrate on the essentials of their job.
The positive feedback from beneficiaries and the certification authority has motivated us to continue our efforts to make Form’App a tool synonymous with quality.
